I authorize the Fort Frances Public Library Technology Centre (FFPLTC) to produce and reproduce digital and analogue photographs, images, audio recordings, video recordings, and derived products, hereafter referred to collectively as "Materials", of me related to my use of FFPLTC Services on the date indicated. I agree to the terms of this agreement for myself and on behalf of any children I can legally represent, if indicated below, hereafter referred to collectively as "Subjects".

The FFPLTC may use in whole or in part, publish, distribute, sell, post on the internet, reproduce and otherwise deal with these Materials as desired without further recourse or duty to all Subjects. All Subjects acknowledge that the FFPLTC is the exclusive owner of copyright relating to these Materials, including negatives, positives and any reproductions produced from these Materials.

All Subjects waive any right to payment for appearance in these Materials. All Subjects waive any right to inspect or approve these or any derived Materials.

If any portion of this agreement is found to be void or unenforceable, the remaining portions shall remain in full force and effect.
